5 Signs You Shouldn’t Ignore a Toothache

By Dr. Gretel Perez Lopez, DMD — General Dentist, Diamond Dental Arts, Punta Gorda, FL

Almost everyone gets the occasional twinge — a quick zing from cold ice cream, a little soreness after a long day of clenching. But some toothaches are your body waving a red flag, and ignoring them can turn a simple fix into a painful, expensive problem. Here are 5 signs you shouldn’t ignore a toothache — and what to do about each one.

1. Pain that lingers or keeps coming back

A brief, fleeting sensitivity is usually nothing to worry about. But pain that lasts more than a day or two, or that keeps returning, is different. Persistent aching often means decay has reached deeper into the tooth, or that there’s inflammation building up. The earlier we catch it, the more likely we can treat it with a simple filling rather than something more involved.

2. Sensitivity to hot and cold that won’t quit

A little sensitivity is common, but if hot coffee or cold water triggers sharp pain that lingers for many seconds after, that’s a signal. It can indicate worn enamel, a cracked tooth, an exposed root, or decay reaching the nerve. Don’t just switch to lukewarm drinks and hope it passes — have it checked so we can find the cause.

3. Swelling in your gums, jaw, or face

This is the one to take most seriously. Swelling — especially with throbbing pain, a bad taste, a pimple-like bump on the gum, or fever — can signal an infection or abscess. Dental infections don’t resolve on their own, and in rare cases they can spread and become dangerous. If you notice facial or jaw swelling, treat it as urgent and contact us right away. Our emergency dentist team can help.

4. Pain when you bite or chew

If a specific tooth hurts when you put pressure on it, that pinpointed pain often means a cracked tooth, a loose or failing filling, or an infection around the root. Cracks in particular are sneaky — they can worsen every time you chew until the tooth splits. Catching a crack early sometimes lets us save the tooth with a dental crown; waiting too long can mean losing it.

5. A constant dull ache or pressure

Not all serious problems announce themselves with sharp pain. A low, constant ache or a feeling of pressure — sometimes worse when you lie down — can point to infection or inflammation deep in the tooth or sinus involvement. Because it’s less dramatic, people tend to live with it far too long. If a dull ache has overstayed its welcome, get it looked at.

Why acting early always wins

Here’s the pattern we see over and over: a small, cheap, quick problem gets ignored because it’s “not that bad,” and months later it’s become a big, painful, costly one. Tooth decay doesn’t reverse itself, cracks don’t heal, and infections don’t clear up without treatment. Addressing pain early almost always means less discomfort, less time in the chair, and less money spent.

What you can do right now for temporary relief

While you arrange a visit, you can rinse with warm salt water, gently floss around the sore tooth to dislodge any trapped food, and use a cold compress on the outside of your cheek for swelling. Over-the-counter pain relievers can help — but don’t place aspirin directly on the gum, and remember these are stopgaps, not solutions.

Prevention is the real cure

The best way to avoid toothaches altogether is consistency: brushing and flossing daily, and keeping up with regular dental check-ups and cleanings. Routine visits let us spot and fix tiny problems long before they ever start to hurt.
