This Notice explains how medical and dental information about you may be used and disclosed, and how you can access this information. Please review it carefully.

Our legal duty

Diamond Dental Arts is required by law to maintain the privacy and security of your protected health information (PHI). We are also required to provide you with this Notice describing our legal duties and privacy practices related to your PHI.

We will notify you as required if a breach occurs that may have compromised the privacy or security of your information.

How we may use and disclose your information

We may use and disclose your protected health information for certain purposes without your written authorization, including:

  • Treatment: To provide, coordinate, or manage your dental care and related services.
  • Payment: To bill and obtain payment from insurance plans or other responsible parties.
  • Health care operations: For office operations such as quality improvement, staff training, and administrative management.
  • As required by law: To comply with federal, state, or local legal requirements.
  • Public health and safety: To help prevent or reduce a serious threat to health or safety, as permitted by law.

Uses requiring your authorization

Certain uses and disclosures of PHI require your written authorization. These may include marketing communications in specific situations and any sale of PHI. If applicable, disclosures of psychotherapy notes also require authorization.

You may revoke your authorization at any time in writing, except to the extent that we have already taken action based on your authorization.

Your rights regarding your health information

You have important rights related to your protected health information, including:

  • The right to inspect and obtain a copy of your health records (with limited exceptions).
  • The right to request a correction (amendment) to your health information.
  • The right to request confidential communications (for example, contact at a different phone number or address).
  • The right to request restrictions on certain uses or disclosures (we are not required to agree in all cases).
  • The right to receive an accounting of certain disclosures, as allowed by law.
  • The right to obtain a paper copy of this Notice upon request, even if you agreed to receive it electronically.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint, and filing a complaint will not affect your care.

Changes to this notice

We reserve the right to change this Notice of Privacy Practices. Any updated Notice will be available at our office and may be posted on our website. The updated Notice will apply to all protected health information we maintain.
